You should rather blame intel. They don't provide V86 mode equivalent beside 64-bit long mode. NTVDM was based on V86 mode - it make simple transitions between RM/PM. But under 64-bit long mode there's no such feature. This means that MS have to completly rework NTVDM as complete VM. But why should do this when there are VPC, Vbox, Vmware...
I'm not a MS fan as you know but I must admit they're quite conservative when they keep NTVDM functionality up to Windows 8 32-bit! They might drop it much sooner. But bacause 32bit supports V86 there's no need extra effort to port NTVDM to newer 32-bit windows (of course with some stupid limitations)...

No way under 64-bit win. Simply use some VM or if you really don't need 64-bit OS (some realy heavy menory consuming apps) then use 32-bit windows (also you may use PAE crack for Vista/W7 to utilize >4GB). Or make multiboot.
