New POSIX, draft 7. C99 doesn't seem to say this, though. Does anyone have a
final copy of the new POSIX standard to check? Maybe this changed between the
draft and final release?

This isn't actually relevant to the ssize_t change, since ssize_t isn't
defined in <stddef.h>. I wasn't proposing that we actually include
<stddef.h>'s definitions in <stdio.h> - it's not a requirement.
