All good comments.  I'll plow through as many as possible before I check
the first version in.  Documentation fixes are really low risk so I'll
do them first.

In some cases the choice of routines was made so the code is compatible
with V2.03 - and in some cases the routines were chosen to minimize the
code footprint size (allows non-heroic ways to make libc dynamic without
needing most of libc to load itself).
