Been a while, but the two biggest issues I recall were weak/missing
support for offsetting the tool from the line (you want the edge of the
end mill to trace the outside of the line, not follow the center of the
line), and there was something just not obvious about taking a .pcb file
for a panel and isolating "elements" in the outline layer you wanted to
mill.  It really just seemed like the code was written to do isolation
milling of traces, not what I was trying to accomplish.
