I found what = is causing the fault.  If you use the either pinlabel ssymbols in a = schematic that fault or warning occurs.  See attached screenshot = for the symbols.  They are under the inputs = directory.

Yes. Those are for = hierarchical design, where the refdes needs to match a pinlabel = attribute on a higher level symbol. You apparently aren't using them in = that manner.
