Wrong list I guess.  Anyway, first you shouldn't be using kernel 
2.0.36pre15 either go to the (now unsuported) 2.0.3?8? or 
upgrade to 2.2.  Second looks like you need to specify the 
base address of the card, ie io=0x300.
