I was thinking it might be to do with the Insert key, the bug is very rare to appear on my system, I've seen it about 3 or 4 times at the most. The only guess I can make is that it's when I'm typing something in FAST and I accidentally hit the Insert key switching to Overwrite mode then when I switch back to Insert by pressing the Insert key again.
