Your book is in error.  As long as C++ has had a standard, this has been
#include <iostream>
Notice
  (1) The angle brackets and
  (2) The absence of '.h'
Your book also probably forgets telling you that such things as 'cout' are 
in the 'std' namespace and so must have a 'std::' prefix or an appropriate 
'using' statement.
