I assume, you are using gcc 2.95.2 and a patched version of GRX.
If my assumption is correct, my speculative followup ;) may help.

The patches described in the FAQ were updated very recently by Ian Miller. 
One of the patches adresses problems with GR_shift_scanline.
Applying the new patches should solve your problems. (You must
patch a freshly installed grx, and not your already patched version.)

BTW. The grx test programs you mentioned should work in 256 color
mode. To try this your could type

testprog 640 480 256

at the DOS-prompt.
