Check the /etc/inittab file and make sure the following lines are in it,
and not commented out (well, the bottom one anyway):
# Things to run in every runlevel.
ud::once:/sbin/update
There was a version of the initscripts package that removed them, because
the kernel was supposed to do it already, but that turned out to not be
true. So, upgrade to a more recent initscripts (if the above lines are
missing) and problem is solved.
