Take a look at the LTC7860 for some ideas of what is needed.
Page 28 has a 28V Nom. Vout surge stopper example.

Also be aware of "AN88 - Ceramic Input Capacitors Can Cause
Overvoltage Transients".
Some transants can be self-inflicted.  Don't know if it applies in
your design or not.
