4a: I have not used it yet ... (MMMV, My Mileage May Vary ;-).

4b: Back annotation needs to be implemented properly first, and probably 
in the way you have demonstrated ;-)
I work strictly from schematic --> pcb.
I do not do schematic <--> pcb by scripts.
When I notice any reason for back annotation I change the schematic 
first before continuing laying out the board.
Can I coin the phrase "pre-emptive back annotation" ;-)
