So in my case I want a couple of holes in the paste stencil to line up with a couple of unplated drills. So I make a new layer, set skip-print to 1, and set merge to toppaste, then add a couple of zero length traces that line up with the drill holes. I have another layer called fab-notes where I have graphics and notes, such as an arrow with text, "exposed copper ok", or "gold plated fingers", etc. PCB::merge is set to fab on this layer.
