In the email you send to the fab, tell them it needs to be countersunk ;-) (in theory, you could have a separate layer named "countersink" in which you put a copper circle around the hole, of the appropriate final largest diameter of the countersink, to keep track of them, but it's still a matter of you and the fab agreeing on what to do)
