Hi-

Presently, the behavior of pcb (when the referenced option is set) is to
include the contents of the "outline" layer on all copper layers in the
output. If the outline layer is empty, then the rectangle defining the
working area is included instead (this rectangle is defined in the
preferences dialogue).

The outline is not included on non-copper layers, including silk, solder
mask, solder paste, etc. Are you requesting that the outline be included on
these layers as well? Or are you not seeing the outline on copper layers?
