What is best way of controlling poligon fill ?

Especially for :

1. preventing the fill in unwanted thin tubs and crevasses

Only way I found so far is to draw track with clearance at the inner
edge of needed area and then filling the clearance between that track
and contained poly with another track. Cumbersome and results are often
not exactly as wanted.

2. Filling otherwise unreachable areas within the polygon

When I do for example polygon fill on top (GND) and bottom, I can
connect otherwise isolated areas with via to the fill on opposing side.
But thing doesn't fill areas it can not reach within the polygon plane,
so that areas are left empty.

Another polygon doesn't help, since polugon within polygon seems to be
ignored. Only solutions seems to be a hole and then another polygon
within it.
