This is how I achieved solder mask windowing in 2012 in gEDA PCB

Nowadays I would simply draw polygons, lines or arcs on the negative solder
mask layer in pcb-rnd to create apertures anywhere on the board, or in the
padstack editor dialogue to create custom mask apertures on a padstack.
