Draw the pad as per the desired footprint and call it Pin A or 
something, then design a component with all the pins in the normal way 
plus pin A.  On the schematic connect pin A to ground.  That's what i 
did on a bluetooth RN42 with the four bits on the corners that were 
suggested as ground. Ian.
