Yes.

No.

A DOS program certainly _starts_ as a 16-bit program (since that's the
only kind DOS knows how to run), but it can switch to 32-bit protected
mode right after that, and run most of its code as 32-bit.  That's
precisely what DJGPP programs do: the startup code begins as 16-bit,
then switches to 32-bit protected mode using a DPMI function.
