Time. Family. Job. Crisis of the day.

This is the "second pass" of the refresh 2 zip based on comments on the
developers discussion list. I believe it's been tested once. Needs
further testing - if OK we'll upload it.

There are a total of 6 files different, all text files. Two .h headers
to get rid of gcc 3.x warnings; a few readme.1st documentation fixes;
djgpp.env upgrades (beware if you've edited it), and manifest/dsm files.

I know, it's been on my todo list since February. Feel free to test it
and let us know how it works. (The file has been there since June 10,
but I didn't have a chance to announce a beta).
