You can use whatever code you like from my own graphics library:

File SLOW.C has putpixel and getpixel functions for the 4-plane
mode. The functions are called write_pixel4p() and read_pixel4p()
They are divided into two parts: one part that does putpixel and
getpixel from/to the screen, and one for in-memory 4-plane bitmaps.

File BMP4P.C has a full-featured blitter.
