Someone has to test this first: it's not at all clear that NTVDM will
not crash when nested programs are invoked via v2loadimage.

And, as you say, there's quite some coding involved, since v2loadimage
in its current shape is suited for debugging, and is very intrusive
(hooks all kinds of interrupts, including Int 31h itself, installs
exception handlers, etc.)
