cr2 is the address which caused the page fault; 0x10000000 is the
base of the DPMI memory space.  So someone referenced address 0.
This is one of those cases that CWSDPMI has more information than
it is allowed to pass back to the client by the DPMI 0.9 specs.
The DPMI 1.0 specs provide for a method to return this info.
