Johan - perhaps it may be worth trying Anders Norlander's Win32 API
with RSXNTDJ so that we don't all have to experience downloading
several megabytes from Microsoft's site.  I know it's intended for use
with mingw32 but given that both packages are based on gcc, it could
save a lot of time - the header files probably won't even need to be
patched.

(The site is http://www.acc.umu.se/~anorland/gnu-win32/ but you
probably already know this!)
