All of the above (and then some) is explained in section 12.2 of the
FAQ, which allegedly ``didn't help much''.  Maybe the person with the
problem would like to describe what did they tried to do following the
FAQ's advice, and how that didn't help.  Then we could try to help
better, instead of just reiterating what the FAQ already says.
