Great! Ove Kaaven has expressed some interest in looking into the
linker/output format issue, and assuming he can get that sorted I will
have a go at making an actual /AF driver. I'll announce it here as soon
as I have something useful, at which point the more people who can
modify it to support their particular cards, the better!

That won't help with making an /AF driver, though, because this is an
externally defined format that we need to comply with. It is very simple
(just a binary block of code), but I think some hackery may be required
to get the djgpp output into the right form. Not that I know anything
about this side of things: I am really just guessing here...
