Menu items have two forms:

* arbitrary text: node name[\n\t,.]
* node name::

Cross references have similar forms except that they begin with "*note"
instead of "*".

So node names could have `:' in them, but not `::'. Also periods should
not appear in node names, because `(info.inf)Menus' explicitly says that
menu items of the first kind may be ended with a period.
