Please also post the message that was printed when the fonts couldn't
be generated.  It can help understand what went wrong.

Which texi2dvi did you use?  You should use the version from the TeX
distribution, not the one from Texinfo.  If you run on Windows 95, you
will need a patch to texi2dvi (mail me).

This document works for me like charm.  No errors, no warnings.

These fonts are all in the distribution.  Please see that you have
these files with a .mf extension somewhere below %DJDIR%/share/texmf.
