Did you read section 18.9 of the DJGPP FAQ list?  It explains these 
issues at some length.  The FAQ is available as v2/faq210b.zip from the 
same place you get DJGPP.

That's the $64,000-question.  Each DPMI server does it differently.  
IMHO, you don't need to know all the gory details, it will be enough to 
read the DPMI spec (the FAQ includes a pointer to places on the net where 
you can get it).  If you *do* need the gory details, you will have to 
look inside the CWSDPMI sources (most of the other servers don't come 
with free sources).
