Because the standard game port interface only supports two.
(Joysticks like CH Flightstick Pro really uses both joystick
interfaces, e.g. the throttle is the Y axis on joystick 2, button 3 is
button 1 of joystick 2, etc.)

Maybe with special hardware and software.

Sound Blasters support two joysticks (the reason Flightstick works).
You just need to buy a Y-cable from Creative.
