I tried to minimize differences from 203 unless it was a bug fix,
or easier just to take the entire 204 file with numerous interlocked
changes.

There is no testing on this (yet) - I thought I would let people who
want to review the changes comment (i.e. you left out a bug fix, this is
too risky to be in a product without more testing, etc).  The _creat
files are not the new/reversed versions either, but just a merge from
what I had and the 2.03 (removing fat32, etc).

They all compile clean with -Wall with the 2.03 headers, which is about 
all that was done at this point.  Comments welcome.
