Should 2.03 refresh items be included separately in the 2.04 documents 
when it's released (they are currently in the wc204)?  I need to 
decide the structure and contents of both releases when I decide
where to edit.  For example, if I edit wc203.txi and put the new
features at the bottom (no separate menu item) and clean up the forward
references in a branch.
