** means pointer-to-pointer. So, ptr would point to a memory location that
points to another memory location. You can also have three or four or more of
those:

long*** ptr;

Now you have a pointer to a pointer that in turn points to another pointer.
Usually, you don't use the *** and rarely use **.

Yes. That's a reference to a pointer. You won't see something like long*&& or
something, though. But here too you may use stuff like long**&.
