You can use other DPMI servers (see the FAQ for details), but AFAIK 
DOS4GW is not a DPMI server, it's a DOS extender that doesn't include 
DPMI services.

No.  Try http://www.delorie.com/djgpp/doc/ug/, that's as close as you get 
to a book.
