Please try this in plain DOS, and without Bash.

This file works for me. After Ispell exits, I have both `test~' and
non-empty, corrected `test'.

If running Ispell from COMMAND.COM instead of Bash doesn't help,
please try to play with the values of these variables:
> TEMP=/TEMP/
> TMP=C:\TEMP
> TMPDIR=C:\TEMP
Also, what does the [ispell] section of your djgpp.env say?
