The following might help:
40 Mhz 386 without '387
DOS 6.20 4DOS 5.0
4 Meg of memory
No TSR's or funny device drivers except for
HIMEM.SYS and EMM386.SYS
The problem definitely is dependant on the order in which programs
that use DPMI services are run.
If INFO.EXE is the first program to use DPMI, the crash occurs.
Running GCC or MAKE or GREP before INFO seems to prevent the crash.
Explicitly loading CWSDPMI: I thought this helped, but without
logging *everything*, I can't be certain anymore. Too confused.
I obtained my copy of INFO.EXE from Delorie.com's web interface
that allows individual files to be downloaded from the .zip file.
It has length 155648.
