In V1.x with non-DPMI, the stack is dynamically allocated.

In V1.x with DPMI, and V2.0 (always DPMI) the stack is a fixed size.  The
default size is 256k; this can be changed by stubedit'ing the image, or
by initializing the global variable _stklen to a constant.
