As mentioned by several posts, Intel offers hardcopy of the DPMI
specs.  These were out of stock for a while, but I received my copy of
the DPMI 0.9 and DPMI 1.0 specs April 26.  You can request them from
Intel's Literature Distribution Center, 800/548-4725 (or presumably
any Intel sales office).  They are also said to be available on
Quarterdeck's bulletin board, (310) 314-3227, in /pub/memory (along
with the VCPI specs).
