Feel free to send a patch -- see <http://cygwin.com/cvs.html> for how to
get the CVS sources (replace "src" by "cygwin-apps" in CVSROOT, and check
out the "cygrunsrv" module).

Well, that's what cygrunsrv is for. You write a console program in perl
(or anything else, for that matter), then install it as service using
cygrunsrv. ;-)
If you meant actually creating and installing a service yourself, look at
Rafael's answer.
