An insecure mail client (i.e. Outlook) is enough. One of the holes it had is that it would open audio attachment automatically just by having the message appear in the preview pane. Unfortunately, the thing was so stupid that "open" would open _any_ type of file based on the file extension, including an executable, even when the content-type was audio of some sort.
