Ron,

First off, you didn't seem to run "cygcheck -svr", as it's missing at
least the list of installed packages.  Also, from your cygcheck output:

  678k 2001/05/21 C:\cygwin\bin\cygwin1.dll - os=4.0 img=1.0 sys=4.0

    Cygwin DLL version info:
        dll major: 1003
        dll minor: 2
        dll epoch: 19

This doesn't look like a very recent version of the Cygwin DLL.  I also
don't see the output of "id", which indicates an old version of cygcheck.
What version of Cygwin are you running?
