Cygwin1.dll is part of usertools.exe.

Install first usertools.exe

Extract lesstif-0.88.1.Cygwin-B20.1.tar.gz to /usr/X11R6.4

Extract X11R6.4-Cygwin-B20.1.tar.gz to /usr/X11R6.4

Extract ddd-3.1.4.tar.gz to /usr/ddd

Add /usr/X11R6.4/bin to your PATH

Add the following to your Windows Environment

SET XKeysymDB=c:\usr\x11r6.4\lib\x11\XKysymDB
SET XErrorDB=c:\usr\x11r6.4\lib\x11\XErrorDB

Go to /usr/ddd/bin directory and type ./ddd.
That is it.

PS: not sure if GDB is part of usertools.exe, if not then you need to
install full Cygwin.
