OK, I'll answer my own question.  Of course I was missing
something.  A quick scanning of pcb source code showed that the
solder mask opening of zero is a problem.  I changed the 'Msk'
value for my paste pads to the same value as the other pads, and it
works fine.

I'm planning to try out Ryan Ohara's laser-cut Kapton stencils.
Has anyone else here used this service before?
